In the medical notes for a child who will be checked into our kids/youth rooms please include any dietaries, allergies, anaphylaxis, emergency plans or other conditions your child may have. This information will appear on the Child's Check-In sticker.
I give permission for Kardinia Church to use photographic and digital images, video and audio recordings of my child for use in the following ways: publications, newsletters, campaigns, websites, social media platforms, live streaming and broadcast, or other related methods of promotion, any future editions and variations of the above, and both electronic and printed formats of the above.
I release Kardinia Church employees, leaders and representatives from any liability of unintended or unexpected accidents which might occur during participation in the program/events.
In the event of accident or illness to my child, I give permission to obtain medical assistance or treatment as may be necessary and to engage any medical professionals (including ambulance and hospital) and I agree to pay for those expenses incurred.
In relation to children under the age of 18 years old, there may be occasions when it is necessary to transport children or to walk to nearby facilities and I give permission to Kardinia Church to facilitate this. I consent to my child viewing G rated movies during an event/program.
